Treasure Dive


		Fourth Annual Klein Scuba/ Timber Bay Northern Wisconsin Treasure Dive September 14^th, 2008 You are registering for the Fourth Annual Klein Scuba/ Timber Bay Northern Wisconsin Treasure Dive, to be held September 14 at Lake Tomahawk . The hunt will be from 10:00 AM until 2:00 PM with the specific dive sites to be revealed at a 9:30 AM briefing. All participants must either register at Klein Scuba or Timber Bay in person or by mail before September 11th. There will be no limit to the number of participants. When registering by mail, please detach and complete the registration form and diver release form on the back of this sheet and mail it along with a copy of your diver's certification card and the registration fee of $25.00 to Klein Scuba. Each diver must fill out a separate registration form. As this dive is strictly boat diving, each participant must make his or her own boat arrangements. Klein Scuba/Timber Bay has reserved pontoon boats through Lakeside Marina. To reserve a spot on a pontoon boat please submit an additional $35 with your registration form. Following the dive, there will be a free picnic for divers, along with the prize ceremony after the meal. Non-divers are welcome and may enjoy food and refreshments for $10 a person. 	TRAVEL/EXCURSION VOLUNTARY RELEASE, WAIVER, AND ASSUMPTION OF RISK Please read carefully and fill in all blanks before signing. I, ______________________________, hereby affirm that I am voluntarily engaging in the recreational activities planned for my trip to Lake Tomahawk, which activities may include, but are not limited to, scuba diving, snorkeling, and boating. If I engage in scuba diving, I affirm that I am a certified diver or a student diver under the control and supervision of a certified scuba instructor, and that I thoroughly understand the hazards of scuba diving. I further affirm that I am thoroughly familiar with the hazards of boating, whether or not said boating involves scuba diving or snorkeling activities. By signing this release, I certify that I am fully aware of and expressly assume all risks involved in scuba diving, snorkeling and boating.
